11.30 Ergonomic Principles with the dental microscope One of the most important revolutions in modern dental treatment has been the introduction of the dental microscop. This introduction has an important influence on the configuration of the working place, the right position of the patient, the dentist and his assistant. We must adapt our suction technique and our work with instruments when working with the microskop. In the lecture we describe the following topics: 1. Configuration of the working place and the microscop. Arrangement of the instruments around the working place and the microscope (suspendet tablet and working desk behind the patient) 2. Correct patient position for treatment in lower jaw and upper jaw. Different positions of the patient´s head during work without and with the dental microscope. 3. Right position of the dental assistant and the dentist (12 o`clock position; 9 o`clock position). The significance of close and wider instrument availabelity. 4. Special suction technique for the assistant to prewent the mikroskop from blurring with steam and spray. 5. Scheduls and steps of working with instruments under the mikroskop with one or two assistants. Five steps of rational instrumentation. |

Sat. 15.00 Synergy between Cone-Beam-Computer Tomography and Microscopy in Endodontics Lecture description: The aim is to visualize the three dimensional shaping, cleaning and filling of the root canal space by innovative in-vitro and in-vivo techniques. Printing the 3D endodontium in your mind in combination with the real time magnified image through the surgical microscope will create a virtual endodontic environment which increases the awareness of the complexity and the final outcome of the treatment and prognosis. Novel non-destructive imaging and visualization techniques like X-ray micro CT ex-vivo (Resolution of <1 µm), Cone beam CT in-vivo (Resolution of 125 µm), Surgical microscopy and Environmental FE-SEM allow us to study the complexity of the root canal system and the tenacity of the biofilm. Dynamic rinsing with RinseEndo of Durr and/or the Photoactivated disinfection (PAD) with laser therapy is a smart concept to reach those bacteria in hidden niches, anastomoses, dentinal tubules and biofilms. Application time, penetration depth, bacterial type, biofilm type, planctonic stage or sheltered stage, all seem to play a role in the success of whatever root canal disinfection techniques. Finally, one could question if Sealing the root canal system by hybridization and resin tag formation, combined with entombing remaining microorganisms, is a dream or reality? The insight in dentin bonding for adhesive restorative dentistry is now finding its way inward and downward the root canal. In order to allow hybridisation and tag formation, again the smear layer, smear plugs, dentin debris and biofilms have to be removed and the system sealed through thermoplastic obturation. Learning Objectives. At conclusion, participants should be able to: Create their own virtual 3D insight in the complexity of the root canal system. Link the imaging concepts to preparation and obturation goals. Understand the way how a biofilm is structured and can be destroyed. Improve the outcome of the endodontic treatment by a 3D seal of the system. |

16.15 Growing old with endodontics? Abstract: The silver-haired Society is upon us; confident, informed, affluent and with unprecedented expectations for vigorous life and supportive healthcare. Dentate and wishing to remain so, they bring the legacies of a lifetime of disease and intervention, and test the diagnostic, planning and operative skills of generalists and specialists alike. Are our societies likely to grow old with endodontic needs? Are you growing old with the endodontics you’re expected to perform? Despite a general improvement in dental health, apical periodontitis is on the increase, and particularly associated with heavily restored and previously root canal treated teeth. But is this important; does it all need treatment, can it all be successfully treated, and are our ageing populations at risk of local and systemic complications? Riding on the tidal wave of technology, the endo enthusiast may be at risk of offering root fillings without restorative strategy, and patient long-term interests. For the strategic and restorable, the challenges of case-winning infection control may be great, compounded by reactive, reparative and degenerative changes in the ageing ‘sclerotic’ pulp space. Though far from invincible, dentists have been empowered by a revolution in lighting, magnification and tools to identify, negotiate and gain control of pulp spaces which were previously beyond the skills of the gifted elite. This lecture will review general and local issues in managing the endodontic needs of an ageing population, highlighting the need for realistic treatment planning and the rational use of contemporary technology.
